By completing and submitting this survey, there is one promotion for which you will be eligible.  This is the 2022 Memorial Tournament $250 Merchandise Gift Card promotion described below.

2022 Memorial Tournament $250 Merchandise Gift Card Sweepstakes

Upon successful completion and submission of this survey, you will be entered into the 2022 Memorial Tournament $250 Merchandise Gift Card Sweepstakes (the “Sweepstakes”).  The terms and conditions of the Sweepstakes are as follows:

Official Rules

 NO PURCHASE NECESSARY TO ENTER OR WIN.
A PURCHASE OR PAYMENT WILL NOT IMPROVE YOUR CHANCES OF WINNING.

Eligibility.  The Sweepstakes is open to all individuals who are legal residents of the 50 United States and District of Columbia and who are at least 18 years old at time of entry. Members of the working media that were credentialed for the 2021 Memorial Tournament Presented by Nationwide are not eligible. 

Where Void.  The Sweepstakes is void outside the USA, including in Puerto Rico, the US Virgin Islands and other United States territories and possessions. 

Sponsorship.  The Sweepstakes sponsor is the Memorial Tournament (“Sponsor”), located at 5750 Memorial Drive, Dublin, Ohio 43085.  Sponsor and its affiliates, agents, representatives, officers, directors and employees, and any entity involved in the development, production, implementation, administration, judging or fulfillment of the Sweepstakes, including without limitation the immediate family members of any such individuals, are not eligible to participate.  Sponsor will conduct the Sweepstakes substantially as described in these Official Rules.

Entry Period.  The Sweepstakes entry period begins on Wednesday, June 9, 2021 at 12:01am EDT and ends on Friday, June 18, 2021 at 11:59pm EDT (the “Entry Period”).

How to Enter.  During the Entry Period, you may enter the Sweepstakes by following the instructions to electronically complete and submit this survey located on Sponsor’s website at https://www.surveymonkey.com/r/2021MemorialTournament (“Sponsor’s Website”).  As indicated above, you are being asked to provide limited contact information, consisting of your name, email address and cell phone number.  (NOTE:  You must complete all required fields of the entry form with information that is valid on the date of the drawing to be eligible.)  All entries become the property of Sponsor and none will be acknowledged or returned.

Limitations on Entries.  THERE IS A LIMIT OF ONE ENTRY PER PERSON PER EMAIL ADDRESS.  No third-party entry or entry through any sweepstakes service is permitted.  Any person who attempts or otherwise encourages the entry of multiple or false contact information under multiple identities or uses any device or artifice to enter or encourage multiple or false entries, as determined by Sponsor, will be disqualified.  The use of robotic entry devices is strictly prohibited.

Sweepstakes Grand Prize.  There will be one Sweepstakes grand prize.  There will be no other Sweepstakes prizes.  That grand prize is a $250 merchandise gift card valid at the 2022 Memorial Tournament golf shop locations. Accordingly, the grand prize is valued at $250.  The grand prize does not include admission to the 2022 Memorial Tournament Presented by Workday.

Drawing.  The winner will be selected in a random drawing from among all eligible entries collected during the Entry Period.  This drawing will take place on or about Friday, June 25, 2021.  The drawing will be conducted by Sponsor or its representative, whose decisions and interpretations on all matters relating to the Sweepstakes and these Official Rules are final and binding in all respects.  Sponsor reserves the right to disqualify any entrant or winner and may refuse to award the grand prize to a person who is not eligible or has violated a rule, gained unfair advantage in the Sweepstakes or obtained winner status using fraudulent means.  If there is a dispute about the identity of the winning entrant, the grand prize will be awarded to the natural person to whom the email address is assigned by the internet service provider responsible for the domain associated with the submitted email address.  If the winner is found to be ineligible or not in compliance with Sweepstakes rules, the winner will be disqualified and an alternate winner will be awarded the grand prize in a separate random drawing.  The winner will be notified by telephone.  The inability of Sponsor to contact the winner by telephone within seventy-two (72) hours after selection may result in disqualification of the winner and an alternate winner may be selected in a separate random drawing.

Odds.  The odds of winning the Sweepstakes will depend on the number of eligible entries received by the Sponsor.

WARNING.  ANY ATTEMPT BY ANY PERSON, WHETHER OR NOT AN ENTRANT, TO DELIBERATELY DAMAGE, DESTROY, TAMPER WITH OR VANDALIZE ANY SPONSOR-OWNED WEBSITE OR MOBILE APPLICATION OR RELATED SOCIAL NETWORKING SITE, THE ENTRY PROCESS, OR OTHERWISE INTEREFERE WITH OR UNDERMINE THE LEGITIMATE OPERATION OF THE SWEEPSTAKES, MAY BE A VIOLATION OF CRIMINAL AND CIVIL LAWS AND SPONSOR RESERVES THE RIGHT TO SEEK DAMAGES AND DILIGENTLY PURSUE ALL REMEDIES AGAINST ANY SUCH PERSON TO THE FULLEST EXTENT PERMITTED BY LAW.

General Terms.  By entering the Sweepstakes, you promise to abide by the Official Rules and decisions of Sponsor, which will be final and binding in all respects.  Sponsor will not be responsible for any injury, damage or loss of any kind arising out of your participation in the Sweepstakes.  YOU AGREE TO RELEASE, DISCHARGE AND HOLD HARMLESS SPONSOR AND ITS AFFILIATES, AGENTS, REPRESENTATIVES, OFFICERS, DIRECTORS AND EMPLOYEES FROM AND AGAINST ANY OR ALL CLAIMS, LOSSES, INJURIES OR DAMAGES RESULTING FROM PARTICIPATION IN THE SWEEPSTAKES.  Except where prohibited by law, by accepting the grand prize, the winner grants Sponsor a perpetual, worldwide, royalty-free license to use the winner’s name, photograph, voice and/or likeness without further authorization, compensation or remuneration of any kind for advertising, promotion and other publicity purposes in any and all media now or hereafter known throughout the world.

Winner’s Name.  The name of the winner will be available within twenty-one (21) days after the close of the Sweepstakes at the end of the Entry Period.  The name of the winner will be posted on Sponsor’s Website.

Agreement to Terms and Conditions.  By the completion and submission of this survey, you agree to these Official Rules containing the terms and conditions of the Sweepstakes.

Limited Use of Contact Information. Your email address and/or cell phone number will be used to communicate with you regarding the promotions described herein. 

Email Authorization.  By clicking this box, you authorize the Memorial Tournament to use your email address to send you Memorial Tournament marketing materials, tournament information, etc.

Cell Phone Authorization.  By clicking this box, you authorize the Memorial Tournament to use your cell phone number for text messages/calls regarding Memorial Tournament marketing materials, tournament information, etc.
